UPMC Western Maryland will serve patients with chronic conditions that over utilize the hospital and/or ED services using telemedicine technology to monitor and report their health statuses at specific intervals.  The goal of the proposed remote patient-monitoring program is to improve the health outcomes and quality of life of low-income and underserved patients with chronic conditions living in rural Allegany and Garrett counties through telemedicine.
