Volunteer Application
If you are interested in applying to become a UPMC Western Maryland volunteer, you can complete the volunteer–adult or teen–application online by clicking on the following links. Please note, teens must be between ages 15 and 17.
Orientation
New Volunteer Orientation is provided via this website to allow new volunteers to readily have access to information necessary for meeting orientation requirements for placement. This orientation must be completed prior to the first day of your placement.
Important Documents
Please read and complete the following to be considered for a volunteer position at UPMC Western Maryland.
If you are under the age of 18, please complete the following with a parent or guardian and return it to the Volunteer Office. Keep a copy for your records.
Health Information
You will need to provide documentation of the current season’s influenza vaccine and Tdap if working with children/infants. Employee Health will order the blood test for TB.
